For your convenience, if you have already scheduled your first session with one of our Rapha counselors, we would like to invite you to download and print your new client paperwork from this web site. All necessary forms are provided in .pdf format (Adobe Acrobat). After you have printed the appropriate forms, please fill them out and bring them to the office with you at the time of your first session.

For Adults:
